- Please fill out a client intake form and health history online prior to your appointment. Forms will be available in office as well. Your massage will not begin until these are complete and waiver is signed.
- All appointments will start on time so please show up 5-10 minutes of your scheduled time
- Cancelation of appointment must be done 24 hours prior to scheduled time or there will be a $30 cancelation fee added to the client's next scheduled appointment.
- Payment can be done online and this is the prefered method of payment. However cash will be accepted in person. No credit cards or checks will be accepted in person.
- This is a professional massage and all Illinois Laws pertaining to massage will be adhered to including proper draping. Any inappropiate behavior, requests, or advances that violate these laws or the established Ethics of massage will result in cessation of the massage and the client forfeits and fees paid. There will be no refunds.
- The client may stop the massage at any point as a result of discomfort or illness
